敗血症性ショック時における血管内皮由来弛緩物質の意義に関する研究

研究概要

敗血症性ショックにおける著明な血管拡張と臓器虚血の進行という複雑な病態を解明する目的で、ラット敗血症モデルより摘出した大動脈、腸間膜動脈、頸動脈および腎動脈において、異常な血管拡張の原因とされている誘導型一酸化窒素合成酵素(iNOS)により過剰に放出された一酸化窒素(nitric monoxide;NO)を、ウエスタンブロット法によりiNOS蛋白の定量を行い、血管平滑筋内のcyclic GMPの値と比較した。その結果、cyclic GMPは大動脈や腸間膜動脈において頸動脈や腎動脈に比し有意に上昇しているのに対し、iNOS蛋白は血管による有意差を認めず、iNOS蛋白とcyclic GMPの値の間に解離があることが判明した。このことは、エンドトキシンショックモデルにおける各血管におけるiNOSは同程度誘導されているにも関わらず、cyclic GMP産生に関しては、血管部位による差があることを意味し、NO以外の何らかのguanylate cyclase活性物質(oxygen radical、あるいはONOO^-など)が敗血症ショックにおいて血管の部位により拡張に差を消磁させている可能性が示唆された。以上の結果は、"Discrepancy between cyclic GMP and iNOS protein levels in rat blood vessel with endotoxemia"のタイトルでJournal of Cardiovascular Pharmacologyに投稿した(平成12年3月)。
来年度は、このNO以外の血管拡張物質の本態を等尺性張カ測定および電気生理学的腫瘍により明らかにしていく予定である。
