前立腺癌の増殖能と転移抑制遺伝子,アンドロゲン受容体遺伝子変異,血管内皮増殖因子

研究概要

パラフィン包埋標本にて昨年度の実績報告書の内容に従ってKi-67を免疫組織化学的に検討した。未治療前立腺癌の針生検標本106例を対象とし、組織標本で最低500個の癌細胞を評価し、Ki-67の染色率を算出し、これをKi-67 labeling indexとした。以上の検索は一人の医師が旅行した。106例のうち標本中の癌細胞が500個に満たない18例を除いた88例におけるKi-67 labeling indexの平均値は13.6%であり、中央値は8.1%であった。Ki-67 labeling indexと腫瘍の悪性度の関係は、Gleason scoreが高い群ほどKi-67 labeling indexが高く、labeling indcxは悪性度と相関が高いことが判明した。Ki-67 labeling indexと生存率の関係を、8.1%以上およびそれ未満の群に分けて検討すると、8.1%以上の群が有意に生存期間が短かった。以上より、Ki-67 labeling indexは前立腺癌の悪性度および予後を反映しており、増殖能を示すパラメーターとして適切であると確認しえた。一方、血管上皮増殖因子については免疫組織化学的方法での染色の適正条件が見っからず現在検討中である。凍結標本を用いるKAI1転移抑制遺伝子の発現蛋白の免疫組織化学的検索およびアンドロゲン受容体遺伝子解析についても、いずれも凍結標本の数が不十分であったこともあり、適正な条件設定ができず、今後標本数の追加を含めて検討課題と考えた。したがって前立腺癌の増殖能と血管内皮増殖因子の発現、KAIl転移抑制遺伝子の発現などとの関係は今回検討できなかった。
