肥満調節因子レプチンによる卵巣機能の調節に関する臨床的・基礎的研究

研究概要

レプチンは脂肪細胞に由来する飽食因子であるが、遺伝的にレプチンあるいはレプチン受容体を欠損するヒトおよびマウスでは妊孕能が欠如するなど、生殖機能にも密接に関与している。一方、痩せた婦人では初潮が遅いこと、過度の体重減少が生殖機能を低下させることなどより、生殖機能の発来や維持には適度な脂肪組織が必要であると考えられている。しかし、レプチンが過剰に存在した場合の生殖機能に及ぼす影響については明らかでない。本研究では、出生後肝臓においてレプチンを過剰に発現するトランスジェニックマウス(Tg)を開発し、生殖機能に及ぼす高レプチン血症の影響を検討した。本年度は以下の成績を得た。
1) Tgの血中レプチン濃度は同腹のnon-Tg(対照群)の6倍高値を示した。
2) Tgの食餌摂取量はnon-Tgの約70%で、体重増加も約70%と有意に抑制されていた。
3) 生殖機能の発現時期を膣開口日齢により検討すると、Tg(n=10)の膣開口(27.7±0.5日)は対照群(29.0±1.0日)と比較して早期に発来した(P<0.05,log-rank test)。
4) 3週齢のTgの卵巣重量は、non-Tgより有意に高値を示したが、子宮重量は差がなかった。
5) 4週齢のTgの卵巣重量は、non-Tgと差がなかったが、子宮重量は有意に大きかった。
6) 8〜11週齢のTgの発情周期(平均4.7日)はnon-Tg(5.4日)と差を認めず、両群とも妊娠可能であった。
7) 11週齢のTgの卵巣及び子宮重量はnon-Tgと有意差を認めず、卵胞発育も対照群と差を認めなかった。
以上、レプチン過剰発現トランスジェニックマウスでは脂肪組織が欠損しているにもかかわらず、性成熟が早期に発来し、妊孕性も維持されていることが示された。すなわち、レプチンは生殖機能調節因子として中心的役割を果たしていることが示された。
