膿胸関連リンパ腫における遺伝子変異の特徴と放射線被ばくの関与

研究概要	甲状腺リンパ腫(TL)のp53,K-ras等の遺伝子変異を"Cold SSCP"法、direct sequencing法にて解析し、膿胸関連リンパ腫(PAL)と比較した。また、TLのmicrosatellite instability(MSI)も検討した。21症例中2例(9.5%)にp53の変異を認めた。(codon190 CCT→CTT,codon272 GTG→GTT)。K-rasの変異は4例(19%)(codon12 GGT→TGT,GGT→GCT,2例 codon13 GGC→GAC)であった。PALでは、21症例中14症例(67%)にp53の変異が見られ、92%にG:C-A:Ttransition、77%にdipyrimidine siteのtransitionが認められたことと比較して、PALとTLの発生に影響する因子には明らかな違いがあると思われる。次に21例のTLに対して、16個の異なったmicrosatellite repeatsをPCR法にて解析した。21例のTLは病理学的にdiffuse large B ce11 1ymphoma(DLBL)10例、follicle center lymphoma 6例、marginal zone B ce11 1ymphoma of extranodal(MALT)type3例、lymphoplasmacytic type 2例に分類されたが、DLBLのみに5例microsatellite instability(MSI)が見られた。これは、他のタイプに対して有意に高い頻度であった。これら5例中4例(80%)がK-ras遺伝子の変異を伴っており、rep1ication error(RER)とK-ras変異の関連性を示唆している。また、これは、"遺伝的不安定さ"がTLのlow gradeからhigh gradeへのprogressionに関与している可能性を示唆している。 PALでは長期にわたる治療薬や細菌やウイルスの産物がp53遺伝子に特殊な変異を引き起こした可能性が、TLでは慢性甲状腺炎という慢性炎症に加え、免疫異常によりRERが起こった可能性が考えられる。今後さらに具体的に、PALおよひTLに関わる因子を追求していきたい。
