糖蛋白質糖鎖不全症タイプIの欠損酵素の同定

研究概要	先天性糖蛋白質不全症タイプI(CDGS-I)は1980年Jaekenらによって初めて報告された常染色体劣勢の遺伝病である。北欧を中心として150例以上の症例が報告されており、国内でも12例が確認されている。その主要臨床症状は精神運動発達遅滞、体幹失調、肝腫大、静脈血栓症皮下脂肪異常、小脳及び脳幹の低形成を特徴とする。我々は、この疾患がアスパラギン-N-結合糖鎖の蛋白部分への転移不全症であることを明らかにした。さらに最近日本人患者の線維芽細胞を解析したところ、マンノース、N-アセチルグルコサミン代謝、転移には全く異常は見られず、ドリコール合成系に異常が認められたことから、デヒドロドリコールリダクターゼの部分欠損を推測した。ところがJaekenらはCDGS-Iの欠損酵素がフォスフォマンノムターゼII(PMMII)であることを報告し、我々の結果と大きく矛盾した。そこで欧米型(Ia)患者線維芽細胞を入手し、日本型(Ib)と比較分析した。その結果Iaでは、PMM活性が健常人の1%前後であり、マンノース-6リン酸/マンノース-1リン酸比が健常人が4〜5:1であるのに対し、15:1と非常に低下していた。またドリコール結合オリゴ糖もMan_<1〜5>・GlcNAc_2という未完成の成分が7割を占めていた。一方、ドリコール生合成に異常は認められずPMMIIの欠損が原因であることが明らかである。これに対し、Ibの5例ではPMMII活性は約30%前後と低めであるが、マンノース-6リン酸/マンノース-1リン酸比は正常であり、ドリコール結合オリゴ糖量も量が少ないが、すべて完成したGlc_3・Man_9・GlcNAc_2のみであった。したがって、CDGS-IaとIbは原因が異なることが推測された。
