肝腫瘍の鑑別診断におけるHepatic tunor indexの有用性

研究概要

1. 我々はカラードプラを用いて肝腫瘍の収縮期最高血流速度と肝動脈の収縮期最高血流速度の両者を測定し、その比をhepatic tumor indexと定義した。このindexのcut-off値を1.0としたとき肝細胞癌と転移性肝癌の鑑別の正診率は82%であった(AJR1997;168:991-995)。今回症例数を増やして検討をした。
2. 未治療で腫瘍径2.0cm以上の肝腫瘍195結節(肝細胞癌93結節、転移性肝癌57結節、肝血管腫45結節)のhepatic tumor indexとgray scale所見を検討した。HCC 93結節中78結節、Meta 57結節中47結節、血管腫45結節24結節に腫瘍血流を認めた。HCC77%(57/78)、Meta9%(4/47)、血管腫8%(2/24)ではHTIは1.0以上であり、HTIの平均値はHCCでは、他の腫瘍に比し有意に高値であった(P<0.01)。HTTが1.0以上の結節では、HCCとMetaの全例で血管造影上明らかな腫瘍血管を認め、IICC10結節と血管腫の全例にAV-shuntを認めた。HTI≦1.0のときHCCである正診率は83%であった。またHCCでHTI≦1.0のとき中等度以上の腫瘍血管である正診率は90%であった。Halo signはHCC90%、Meta91%、血管腫7%であった。HTI≦1.0かつhalo陽性のときHCCである正診率は83%、HTI1.0>かつhalo陰性のとき血管腫である正診率は94%であった。カラードプラでHTIを測定することで、非侵襲性に腫瘍血菅の程度、AV-shuntの有無を推定可能であり、haloの有無との組み合わせで肝腫瘍の鑑別診断の正診率が向上した。
3. 肝細胞癌93結節におけるhepatic tumor indexと血管造影でのvascularityとの関係を検討をした。腫瘍血管の程度を腔高度(明らかに広狭不整で屈曲蛇行する腫瘍血管)、中等度(一部広狭不整で屈曲蛇行する腫瘍血管)、軽度(広狭不整な部位はないが一部屈曲蛇行する腫瘍血管)に分類した。Hepatic tumor indexが1.0以上は78結節中57結節であり、全例明らかな腫瘍血管を有していた(高度31結節、中等度26結節、また10結節にAP-shuntが存在した)。Hepatic tumor indexが1.0未満は78結節中21結節であった(中等度8結節、軽度11結節、腫瘍血管なし2結節)。Hepatic tumor indexは腫瘍血管の程度とAP-shuntの存在の有無と相関すると思われた(Cancer Detection and Prevention in press)。
