心不全の病態形成における内因性エンドセリンの意義の解明-エンドセリン拮抗薬を用いての検討

研究概要

心不全は心機能低下の終末的病態で、その予後は不良である。アンジオテンシン変換酵素阻害薬は心不全患者の生命予後を改善し、心不全治療の第一選択薬となっている。しかしその改善の程度には限界があり、新たな心不全治療薬が期待される。本研究ではエンドセリン(ET)を取り上げた。ETは血管トーヌス、神経体液性因子分泌さらに体液バランス調節に関与し、前駆体BigET-1よりET変換酵素(ECE)により生理活性を有するET-1が産生される。血中BigET1、ET-1濃度は共に心不全患者の重傷度、予後規定因子であり、ET受容体拮抗薬が心不全モデルの予後を改善するとの報告がある。しかし心不全病態下においてET受容体のダウンレギュレーション等が生じ、その効果が確実なものとは言えない。ECE阻害薬によるET系の阻害が心不全治療の新たな一面を築くことができるかを、高頻度右室ペーシングにて作製した低心拍出量性心不全イヌを対象に、ECE阻害薬(ECEI)FR901533の静脈内投与(1.0,3.0mg/kg)を行ない、心血行動態、神経性体液因子、腎体液量バランスに及ぼす効果を、ETA受容体拮抗薬(FRI39317)のそれと比較検討することにより明らかにした。
ECEI、ETA受容体拮抗薬は共に、
1) 血管抵抗の低下を伴って、動脈圧、肺動脈楔入圧を低下させ、心拍出量を増加させた。しかし心収縮力の指標である左室内径短縮率には変化は無かった。その時ECEIの降圧効果はETA受容体拮抗薬よりも小さく、ECEIが血管拡張作用を有するETB受容体の作用を阻害するためと考えられた。
2) 動脈圧の低下にもかかわらず、交感神経活性の指標であるノルエピネフリン分泌を亢進させず、ANP分泌を低下させた。ECEIのみがレニン-アンジオテンシン-アルドステロン濃度を低下させ、心不全重症度に伴って増加するこれら神経体液性因子の賦活化を抑制した。
3) 腎血漿流量、腎糸球体濾過率の増加を件って、尿量、尿中Na排泄量を増加させた。ECEIの腎血管拡張作用は、ETA受容体拮抗薬よりも小さく、全身血管床と同様にETB受容体を介する作用を阻害するためと考えられた。しかしECEIが体液貯留に桔抗する可能性が示された。
以上の研究結果はエンドセリン変換酵素阻害薬が心不全の進展を防止できる可能性を示すものであり、今後ECEIの慢性投与が、心不全治療の新たな展開をもたらすものと期待される。
