がん患者への精神医学的介入に関する神経精神免疫学的研究

研究概要

本研究では、がん患者への精神医学的介入が患者のQOLおよび予後に及ぼす影響について検討し、以下のような結果を得た。
1. 術後の乳癌患者227例について、がんリハビリ評価システム(Cancer Rehabi1itation Examination System:CARES)を用いて術後1年目のQOL予測因子について検討した結果、感情状態の評価尺度(Profile of Mood State:POMS)とリンパ節転移が術後の不良なQOLの予測因子となりうることが示された。
2. 再発リスクの高い乳癌患者126例にグループ精神療法に関して調査した結果、興味あり106例(84,1%)、うち53例が参加意思ありで、高いニーズが示された。参加意思あり群の背景因子として、強い不安、抑うつが関連していた。実際にグループ療法を実施できた10例の分析から、日本人用のグループ療法プログラムを作成し、その妥当性を検討した。
3. 乳癌患者18例について、がんに対するコーピングスタイルの評価尺度(Mental Adjustment to Cancer Scale;MAC)、感情状態の評価尺度(POMS)を、また患者の同居家族全員に対して、家族機能の評価尺度(FAD)を用いて評価し、それぞれの相関を検討した結果、FADとMAC・POMSには高い相関性があり、患者のコーピングや感情状態に家族機能が密接に関与することが確認され、限られた医療資源の中で患者のQOLを向上させるためには、家族の効率的な関与が必要であり、その支持体制の確立が重要であると考えられた。
精神医学的介入と免疫マーカー(NK細胞活性、TNF活性など)との関連は今回の研究では確認できなかったが、精神医学的介入はがん患者のQOL向上に有用であることが示された。
