乳癌の非侵襲的早期診断法の開発

研究概要

乳癌の早期診断は、その後の治療、予後に大きな影響を示し、簡便で客観性のある診断方法の開発が必要である。我々は、乳管、乳頭を介して分泌される分泌液に着目し、その分泌液に含まれるcarcinoembryonicanti9en(CEA)を検出する乳頭パッチ法を開発した。すなわち乳頭にニトロセルロース膜を24時間貼付したのちに、その膜に抗CEA抗体を滴下し、免疫染色を施行してCEAを検出する方法である。
22人の女性健常人および32人の女性乳癌患者に対し乳頭パッチテストを行なった。健常人のCEAの分泌は平均0.6unitsであった。癌患者の患側では、32人中30人で明らかに高いCEA値を示し、平均16.1unitsであった。一方癌患者の健側乳頭での分泌は平均2.Ounitsであった。このように乳癌患者患側から高いCEA値を検出することが可能であることが明らかになった。
問題となる点としては次の2点が考えられる。乳頭に貼る貼付剤の蛋白吸着能、および吸水能力が時間と共に低下する点。CEAが乳輪、皮膚からもごくわずかであるが免疫染色で検出できる程度の分泌がある点。乳頭パッチテストの実用化を目指すにあたって、これらの点の改良が必要であることが考えられた。
