研究概要

東南アジア地域におけるHIVおよび肝炎ウイルス感染症の現状に関する疫学的調査研究とその分子疫学的解析を行い、次の成果を得た。
(1)ミャンマー中部のHIV流行に関する分子疫学的研究
ミャンマーでは,これまで,タイに由来すると考えられるHIV-1サブタイプEとB'が流布することを報告してきたが,今年度の解析によって,あらたにサブタイプCがミャンマー北中部に存在すること,また隣接する中国に分布するサブタイプCと構造的に最も近縁であることを明らかにした。マンダレーを中心とする地域の薬物乱用者の間のHIV感染率は85%と世界にも例を見ない極めて高いレベルにあり,そのような高い感染率と複数のサブタイプのウイルスがco-circulateしている状況を反映して,B'/CやE/B'などの新しいタイプの組換えウイルスが発生していることを見い出した。この知見は,複数のサブタイプ間のsuperinfectionを阻害する免疫学的機構が十分には機能しないことを意味しており,ワクチン開発の困難さをあらためて浮きぼりにするものと考えられる。
(2)B型肝炎ウイルスの迅速ゲノタイピング法の開発とゲノタイプの地理分布に関する研究
ウイルスのゲノタイプを決定することは,病原性・感染経路・治療指針等を探る上で重要である。現在HBVには少なくとも6つのゲノムタイプ(A〜F型)が存在することが知られている。そこで我々はHBVゲノムタイプを迅速・簡便しかも特異的に決定できる方法を開発し,HBVの分子疫学的研究に応用した。アジアではC型が主要なゲノタイプであったが,ベトナム(ホーチミン市)においてはB型との共存係わりの多いのが注目された。ゲノタイプと肝疾患や治療との関連性・変異株出現頻度との関係などを検討中である。
