抗リン脂質抗体の関与する動脈硬化の発症機序の解明

研究概要	抗リン脂質抗体症候群とは、血清中に抗カルジオリピン抗体やループスアンチコアグラントなどの抗リン脂質抗体(自己抗体)が現れる難治性の自己免疫疾患であり、動・静脈血栓、習慣流産、および血小板減少などの臨床症状を呈する。最近、粥状動脈硬化発症の初期に認められるマクロファージによる酸化LDLの取り込みに、β2-グリコプロテインI(β2-GPI)および抗β2-GPI抗体が関与している可能性を示した。今回は、この自己免疫応答が関与する動脈硬化の発症の機序についてさらに解析を行い、また、β2-GPIの認識する酸化LDL粒子上のligandを同定し単離・精製した。本研究で得られた知見は以下の通りである。β2-GPIは、硫酸銅処理による化リポ蛋白に対し特異的に結合した。また、酸化リポ蛋白へのβ2-GPIの結合に伴って、本疾患由来の抗β2-GPI抗体の結合性が認められた。抗カルジオリピン抗体が変性リポ蛋白の一つであるMDA修飾リポ蛋白(MDA-LDL)を認識していることを示唆する報告があるが、β2-GPIおよび抗β2-GPI抗体のMDA-LDLへの結合は全く見られなかった。β2-GPIが結合する酸化LDL分子上のligandがBligh&Dyer extractionによる脂質画分に抽出されることが明らかとなり、それらを単離・精製した。酸化LDLのマクロファージによる取り込みはβ2-GPIの単独添加により減少したが、β2-GPIおよび抗β2-GPI抗体共存下では、逆に有意に増加した。“酸化LDL、β2-GPI、および抗β2-GPI抗体からなる免疫複合体の形成に伴い酸化LDLのマクロファージよる取り込みが増加する"ことを示すこれら一連の知見は、抗リン脂質抗体症候群において免疫異常の関与する動脈硬化の発症機序が存在することを強く示唆するものであり、自己免疫性の閉塞性血管病変の研究に新たな進展が期待される。
