歯周病における生体防御と組織修復に関する総合的研究

研究概要

1.歯周病における生体防御反応の解析:歯周病関連細菌のリポ多糖(LPS)の好中球及び単球に対する活性化機序の解析によるとP.gingivalisとA.actinomycetemcomitansとでは活性化に関与するレセプターに差異がある事が明かとなった。好中球のFcレセプターの遺伝子解析によると貧食能、活性酸素産生能の低下しているNA2/NA2群の方が歯周病に罹患し易い事が明らかとなった。線維芽細胞は、インターロイキン1(IL-1)やLPSの刺激により種々の炎症性ケモカインの活性を制御するCD26/Dipeptidyl peptidase IVを発現する事が明らかになった。更に、線維芽細胞はIL-1やTGF-β、PDGFなどの細胞増殖因子の刺激により高分子量のヒアルロン酸を産生し炎症を制御している事が観察された。又唾液中の分泌型IgA抗体量と歯周病の臨床症状との間に関連性が認められた。更に、炎症性細胞の機能発現に関与するIP3結合タンパク質の解析や抗体産生応答性についてのT細胞サブセットの解析についても重要な研究結果が得られている。
2.歯周病における組織修復の解析:歯周組織再生療法に臨床使用されているエムドゲインを応用とするとアルカリフォスファターゼ陽性の歯根膜細胞の増殖を促進した。又ヒト歯根膜由来線維芽細胞のSPARK分子の発現が歯周組織の破壊の進行に関与している事が示唆された。歯槽骨の吸収及び再生を動物実験モデルにて解析したところプロスタグランディンE_2(PGE_2)の全身投与を行うと歯周炎局所での骨吸収の抑制が観察されるとともにテトラサイクリン併用の組織再生誘導(GTR)法を行うと新生歯槽骨の形成が促進された。更に、ラット歯槽骨の創傷治癒過程で発現量が変化する遺伝子群の検索を行い興味深い知見を得ている。
以上、歯周病における生体防御反応及び組織修復について重要な研究成果を得ており、今後これらの研究を更に発展させると共に臨床応用についての可能性の検討を行う予定である。
