肝癌発生、進展における肝特異転写因子発現変化の解析、病理診断、遺伝子治療への応用

研究概要

肝組織を構成する細胞には、肝細胞及び胆管細胞への両方向に分化する幹細胞が存在すると考えられており、rodentでは、ovale cellと呼称されている。ひとにおいては、肝細胞-胆管細胞混合癌が存在し、肝細胞癌、胆管細胞癌、その移行像を示す癌部よりなる。混合細胞癌の肝癌部分、胆管細胞癌部分、肉腫様部分(幹細胞に相当すると考えられる)の培養細胞を使って、その分化を制御する肝細胞特異転写制御因子核蛋白HNF-4とHNF-4に発現制御されるHNF-1αの発現変化及びその細胞内発現分布をみた。肉腫様部分は、HNF-4、HNF-1αどちらも発現はなかった。胆管細胞癌部分においては、HNF-4の発現をみるが、胆管細胞部分、肉腫様部分には、HNF-1αの発現を認めなかった。肝細胞癌部分においては、HNF-4、HNF-1αどちらの発現も認めた。各培養細胞の細胞内発現分布をみると、胆管細胞癌部分の培養細胞のHNF-4の発現は細胞質にみられるのみで核内での発現は認めなかった。このことから混合細胞癌においては、その複雑な組織像はHNF-4の発現により肉腫部分より肝細胞、胆管細胞との差異があるが、肝細胞部分と胆管細胞部分との差異は、胆管細胞部分では、HNF-4の発現をみるが、HNF-4が核内で発現し、発現制御核蛋白としての本来の機能を果たしているかどうかによっていると考えられた。実際の混合癌の病理組織標本を観察すると胆管細胞癌部分と肝細胞癌部分とその移行像が複雑に入り交じっていることが多い。この微妙な形態的特徴もHNF-4の細胞内発現分布によると考えると、よく理解されうる。現在胆管細胞癌部分のHNF-4の核内移行障害について、核蛋白の核内移行蛋白について検索をすすめている。
