血管壁リモデリングに及ぼすトロンビン/トロンビン受容体の関与とそのシグナル伝達系

研究概要

今回の研究で明らかになったことは以下の通りである。
1. トロンビンはトロンビン受容体を活性化し、NF-kBを活性化する。これはMAPカスケードの活性化を伴うものである。
2. Directional Coronary Atherectony(DCA)で得られた冠動脈硬化閉塞部では血管平滑筋細胞が増殖していたが、この血管平滑筋細胞ではトロンビン受容体の過剰が増強していた。この細胞が合成型か収縮型かは現在のところ不明である。
3. トロンビンは血管平滑筋細胞に遊走を促進した。またこの細胞では組織因子、VEGFの発現が増強していた。
これらの結果から、粥状動脈硬化部位では何らかの原因で血管平滑筋細胞にトロンビン受容体の発現が強まっていることが明らかとなったが、その原因として我々の他のデータから酸化変成しDL,糖化タンパクなどを候補に考えている。
このようにトロンビンはこの過剰に発現したトロンビン受容体の活性化を介して、凝固線溶因子とそのインヒビター類(組織因子、PAI-1)、細胞接着因子(P-セレクチン、ICAM-1,ELAM-1),細胞遊走因子(MCP-1)、サイトカイン(IL-1,TNF-α)、増殖因子(VEGF)など発現させ、血管壁のリモデリングに重要な役割をはたすものと考えられる。
我々はすでにトロンビン受容体からのシグナルを遮断する薬剤を見いだしているが、今後これらの薬剤でアテローム形成が抑制しうるか否かが重要な課題として残った。
