ウイルス下痢症の病態,分子疫学と予防に関する研究

研究概要

下痢症ウイルスの分子疫学的研究を続けた。我が国で昨年度にロタウイルスの9型の流行を札幌で見出したが、今年度は佐賀に認めた。舞鶴、東京、大阪ではほとんどが1型で9型は認められなかった。佐賀では90%(22検体)が9型であったが、その遺伝子の解析および9型のモノクローナル抗体の結果から昨年度の札幌のものとは異なり、1998年ごろタイで見出された株と近いことがわかった。過去20年ぐらいの我が国の成績から、13-15年前に多く9型が見られ1,2年で消失したのが明らかにされ、周期性をもって9型が出現すると推測された。全体的には1型が大多数を占めたが、やや頻度は下降しており、2型も認められた。ワクチンの開発には9型も含める必要が考えられた。アデノウイルスは40<41型で全体の下痢便の10%位に認めた。カリシウイルスがGIIが中心で、アストロウイルスも僅かだが検出された。
東京の1保育園で、1999年6月-2000年5月まで、毎週1日のルーチンの採便と下痢の流行時は続けて採取した。その結果1999年6月と2000年5月にアストロウイルスの流行が見られた。また1999年6月はカリシウイルスの排泄も見られた。また12月にカリシウイルス単独の下痢の集団発生が見られた。ロタウイルスの流行はなかった。カリシウイルス(酵素抗体法)のために、GIIのカプシド蛋白をバキュロウイルス系で発現させた。また同時に、開発中のGI,GII共通抗原を利用した酵素抗体法による抗原検出キットを使用する機会を得た。それを用いると遺伝子増幅で検出したのと比較し、70%が検出できた。国際共同研究として、ベトナムの検体では。下痢症ウイルスのなかでロタウイルスの占める頻度は日本と比較すると多かった。9型は見られず、4型の占める割合が多くなっていた。カリシウイルスはGIIが多かった。
