糸球体機能調節機序及びその進行性腎疾患及び高血圧における意義

研究概要	本年度は腎糸球体輸入細動脈におけるアンジオテンシンII(AngII)の1型及び2型(AT_1及びAT_2)受容体とアラキドン酸のチトクロームP450代謝産物の関係について検討した。アラキドン酸のチトクロームP450代謝産物の主なものは20-ハイドロキシエイコサテトラエノイン酸(20-HETE)とエポキシエイコサトライエノイン酸(EET)があり、前者は血管収縮作用、後者は血管拡張作用を持つ。我々は以前AT_2受容体が輸入細動脈を拡張し、その作用にEETが関与することを報告している(J.Clin.Invest.100:2816-28,1997)。そこで、AT_2受容体拮抗薬(PD)、一酸化窒素合成酵素阻害薬(L-NAME)、EETの合併阻害薬(ミコナゾール)及び20-HETEとEETの両者の合成を阻害する17-ODYAのAngIIの血管収縮作用に及ぼす効果を検討した。PD及びミコナゾールはそれぞれAngIIの血管収縮作用を増強したが、両者を併用してもどちらか一方単独での増強作用と変わりがなかった。ミコナゾールやPDによる増強作用はL-NAME存在下でも認められたことより、AT_2受容体を介する血管拡張作用には一酸化窒素ではなくEETが関与することが示唆された。一方、17-ODYAはAngIIの血管収縮作用を有意に減弱させ、AT_1受容体を介する血液収縮作用には20-HETEが関与していることが示唆された。この17-ODYAの効果はL-NAME存在下でも認められ、一酸化窒素の産生亢進によるものではないと考えられる。P450代謝経路は腎でのアラキドン酸代謝の主要な位置を占めることが最近報告されており、腎血行動態の調節に機能的に重要である可能性が示された。
