新しい屈筋腱修復法と術後療法適用下における血管新生の役割

研究概要

1.新たに開発した屈筋腱縫合法を、屍体腱にて評価した論文を、Hand Surgery第3巻(1998)に掲載した。またin Vivoにて有効であることを生イヌを用いて、これまでの縫合法より簡便であるばかりか生体力学的にも優れていることを呈示した。更に、術後の自動運動療法が固定療法に比較して生体力学的に統計学的に有意であり、組織学的にも良好な治癒過程であることをまとめ、the Journal of the Hand Surgeryに投稿しin pressの状態である。
2.この縫合法を、吸収糸にて行った研究をすすめ、屈筋腱縫合は吸収糸でも可能であることを示した。これまでと大きな相違点は、開発した縫合法が強固な縫合張力を有していること、このため、縫合直後より自動運動が可能となり縫合部に十分なストレスを作用させることが可能となった点にある。この結果をまとめたものも、the Journal of the Hand Surgeryに投稿しin pressの状態である。
3.縫合部におけるlockingとgraspingの相違点をあきらとし、4本糸主縫合を我々の開発したKessler変法と最近アメリカ手の外科学会で有効と報告されたCruciate法において比較を行い、張力その他生体力学的要素に有意差を持って、lockingと我々の開発した縫合法の組み合わせが重要である知見をまとめ、アメリカ手の外科学会で発表したほか、the Journal of the Hand Surgeryに投稿し掲載された。
4.更に新しく開発された縫合糸の評価を行い、我々の開発した縫合に適応することでより生体力学的に有利である可能性を、屍体腱にて評価し、the Journal of the Hand Surgeryに投稿しin pressの状態である。
