頭頚部癌に対する遺伝子導入化学療法の基礎的研究

研究概要	1)CDDP耐性頭頚部癌細胞株を樹立し、その耐性機序を研究した結果、グルタチオンとMRPの協調によるGS-Xポンプの高発現による、細胞内CDDP蓄積量の低下、およびDNA修復能の亢進によることがわかった。 2)口腔・中咽頭癌における癌関連遺伝子の免疫組織化学的陽性と5年生存率の関係を検討した結果、p53の発現は予後が悪く、逆にアポトーシスを誘導するBaxの発現群の予後は良かった。Bcl-2の発現は予後とは無関係であった。 3)p53,Bax,Bcl-2の陽性とアポトーシスの誘導との関係を免疫組織学的タネル法で検討した結果、p53,Bcl-2,Baxの発現とアポトーシスとは有意差は出なかったが、Bax陽性群ではアポトーシスの発現が多い傾向にあった。 4)臨床材料でBaxの発現群の予後が良かったので、bax遺伝子のトランスフェクトをエレクトロポレーション法で頭頚部癌細胞株およびCDDP耐性株に行った。ウエスタンブロット法でその発現増強を確認した後、CDDPに対する感受性をMTT法で検討したところ、両株において有意にbax発現株のCDDPに対する感受性が増強され、死細胞が増加した。 5)アポトーシスを抑制する、bcl-2発現株に、そのアンチセンスDNAを入導したところ、bcl-2の発現が明らかに抑制された。そこで、この株にCDDPを作用させたところ、アンチセンス導入群で有意差をもって生存癌細胞数が減少した。
