肺サーファクタント蛋白質A及びDによる生体防御の分子機構と臨床応用の開発

研究概要

肺サーファクタント蛋白質SP-A、SP-D、および、マンノース結合蛋白質(MBP)は、C末端側に糖鎖認識領域(CRD)を有するC型レクチンのコレクチン・サブグループに属する。コレクチンは、複合脂質や病原微生物をリガンドとし、マクロファージを活性化して肺の生体防御においても重要な役割を果たしている。本研究は、SP-A/Dの構造と生体防御機能発現の分子機構を解析し、臨床応用の開発を目指すことを目的に遂行された。以下に結果を要約する。
(1)マクロファージに発現するCD14は、リポ多糖(LPS)の受容体として働くが、SP-AとSP-DははともにCD14に結合してLPSとCD14との相互作用を変化させることを見いだした。さらに、SP-AとSP-DによるCD14結合の分子機構が異なることを明らかにして呼吸器の生体防御機構におけるSP-AとSP-Dの重要性を示した。(2)SP-AとSP-Dと同様にMBPもCD14に結合することを見いだし、CD14がコレクチンのリガンドとして機能することを示した。(3)ARDS患者のBAL中SP-Aは低下するが、SP-Dは低下しないことを明らかにした。ARDS at riskの患者ではSP-Aの低下しない例ではARDSに進展しなかった。(4)膠原病合併間質性肺炎患者の血中SP-AとSP-Dの測定は、CTでのみ検出可能な軽微な病変を検出できることを示した。(5)SP-AとSP-DをpEE14 vectorにクローニングし、CHO-K1細胞にトランスフェクトしてGlutamine Synthetase系による大量発現系を確立した。(6)感染を惹起した糖尿病発症ラットではBAL中、SP-AとSP-Dが増加し、肺コレクチンが急性期反応物質として機能することが示唆された。(7)SP-AのCys204からC末端側領域を相同なSP-D領域に置換したSP-A/SP-Dキメラ体の検討から、肺コレクチンのC末端側の25アミノ酸残基がリガンドとの相互作用に重要であり、感染防御分子としてこれらのキメラ体を用いることが有用であることが示唆された。
