重症型魚鱗癬の発症機序の解明と新しい診断ならびに出生前診断法の開発

研究概要	1)各家系ごとのlinkage analysis 患者家系内の発症者、健常者の可及的多数の末梢血からDNAを抽出し、ヒト遺伝子地図上に設定されたmicrosatellite markersについてhaplotypeを検索、患者家系内でlinkage analysisを行った。特に、原因分子をコードしている可能性の高いepidermal differentiation complex(EDC)領域については、優先的にmicrosatellite markersを設定した。この結果、各病型、各家系について、ある分子については病因との関連が否定され、また、他の分子については病因との関連の可能性が示唆された。これらのデータと、前年度に行った臨床的検討、免疫電顕を含めた形態学的研究のデータを総合し、各病型、各家系ごとに原因遺伝子を推測した。 2)遺伝子変異の検索 PCR法により、標的遺伝子のgenomic DNAを増幅し、1塩基置換でも検出可能なheteroduplex解析により、遺伝子変異を有するDNA領域をスクリーニングした。最終的に、その領域の塩基配列を決定することにより遺伝子変異部位を同定し、さらに、制限酵素消化、あるいは、allele specific oligonucleotide hybridization法(ASO法)により、遺伝子変異の確認を行った。 3)重症型魚鱗癬における新しい正確な病型組織学的方法で原因分子の発現異常を明らかにする方法、あるいは、原因分子の遺伝子のsequencingにより遺伝子変異を検索する方法で、各病型ごとの正確な診断が可能となった。また、原因遺伝子変異を特定できた家系においては、絨毛膜生検による胎児DNA採取とmutation analysisで早期の出生前診断が可能であるようなシステムを確立した。
