特発性パーキンソン病に対する淡蒼球外節電気刺激治療の開発

研究概要

カニクイ猿(Cynomolgus monkey)4頭に対して一側頚動脈にMPTP(1.2mg/kg)を間歇的に注入し、慢性パーキンソン病モデルを作成した。症状の発現は経時間依存性であり、4週間後の神経症状評価では、Gash's hemi-parkinsonian primate rating scaleでscore15以上であることを確認した。その後、定位脳手術的に4箇所の電極を有する脳深部刺激装置を設置した。尚、電極挿入部位の確認はレントゲン撮影にて行った。有線で単極刺激を用い解析を行ったが、淡蒼球内節においては高頻度刺激(130-160Hz)、刺激幅(90-100μsec)で,従来のヒトでの効果と同様にパーキンソン症状の改善が得られた。また、淡蒼球外節の低頻度刺激(5Hz)、刺激幅(90-100μsec)では各々の個体において上記のGash's scoreで6-7の改善が得られ、高頻度刺激(130-160Hz)では逆に症状の悪化がみられた。また、2個体においては全身麻酔下で、経心的に潅流固定を行い、電極挿入位置の確認、およびMPTP投与による黒質緻密層のドパミン作動性神経細胞の脱落を確認した。現在、統計処理などを含めてデータの解析をおこなっている。
