分子種に特異的なインヒビンの迅速アッセイ法の開発と臨床応用

研究概要	インヒビンのアッセイ法として、抗インヒビンポリクローナル抗体を用いたRIAはインヒビンA、インヒビンB、インヒビンαモノマーとも検出する。私達の用いている抗体はオーストラリアのグループが用いてきた抗体に比べインヒビンBに対する親和性が強く、正常月経周期婦人で、EIAによるインヒビンAとインヒビンBの両者を反映した値を示す。このRIAでtracerや緩衝液の検討により、0.1IU/mlの高感度で、排卵誘発時、周閉経期〜閉経後、顆粒膜細胞腫を含む卵巣腫瘍治療時、などの臨床検体を測定した(IR-inhibin)。顆粒膜細胞腫ではIR-inhibinは臨床経過をよく反映し、腫瘍の全摘出により感度以下まで低下するのに対して、再発例では、他の腫瘍マーカーより早期に血中濃度の上昇を示した。ゴナドトロピンによる排卵誘発時に、OHSSを生じた症例では、インヒビンAやインヒビンBよりより早期にIR-inhibinが高値となったことより、IR-inhibinが排卵誘発時のマーカーとして有用である可能性が考えられた。60歳以上の婦人ではIR-inhibinは感度以下であった。現在、腫瘍のスクリーニングへの適応の検討のため、多数例の閉経後婦人の検体を測定中である。インヒビンAについてimmunofluorometric assayを開発し、妊娠時のインヒビンAについて測定中である。インヒビンBについては作製した抗体ではimmunofluorometric assayで期待された感度は得られなかった。
