頭頚部癌、食道癌に対する微粉末磁性体を用いた磁場誘導組織内温熱療法の研究

研究概要	本研究は頭頸部癌、食道癌に対して磁性体を用いた磁場誘導組織内温熱療法の抗腫瘍効果を検討したものである。磁場発生装置は最大出力10kw、周波数64kHzであり、内径7cmと14cmの誘導コイルがあり、中心で最大450ガウスの磁場が得られる。磁性体を微粉末状にしてさらにカチオニックリポソームで封入したもの(Magnetic Cationic Liposome:MCL)を用いた。まずVX-7移植舌腫瘍に対してMCLを局注し磁場誘導組織内加温法の抗腫瘍効果を検討した。加温は30分の1回加温とした。その結果、腫瘍内は43℃以上が観察され、4週後には治療群においては腫瘍は著明な減少を示し、病理組織学的検討においても、腫瘍細胞はみられなかった。次にMCLの頸部転移リンパ節への移行量を測定し、転移リンパ節への温熱療法の効果を検討した。その結果、注入後3日目のリンパ節内のMCL量が一番多く観察された。よって注入後3日目に頸部に対して磁場を照射した結果、4週後では加温された転移リンパ節転移は腫瘍組織は認めず、対照群に比べて著明な効果が得られた。次に患側の外頸動脈からカテーテルを挿入し、MCLを動脈より注入し舌腫瘍への組織内移行を検討した。その結果、どの時点においても舌腫瘍内にはMCLの移行がみられず、舌腫瘍に達するまで血管壁にMCLが吸収されてしまうことが考えられた。進行性食道癌治療は磁場誘導組織内温熱療法(10分間、2回)とCDDP:7mg/m2x5days,5-FU:330mg/m2x7daysの化学療法を併用した。その結果、ステントの温度は50℃が計測された。結果はCR:1例、PR:6例、NC:3例であった。CR例は病理組織学的にも腫瘍細胞はみられなかった。また摂食困難が解消された症例が5例みられた。このようにステントを用いた磁場誘導組織内温熱療法と化学療法の併用は進行性食道癌の局所の進行を抑制し、進行癌患者のQOLを高めることが確認され、有用な治療法として期待される。
