胆汁逆流による胃癌と食道癌発生についての分子病理学的-UACL関与

研究概要

胆汁を含む十二指腸液が胃に逆流することで胃癌と食道癌が発生するが、本研究は、十二指腸液逆流が粘膜上皮にどのような影響を及ぼし発癌に到るかを形態学的ならびに分子病理学的に解析したものである。逆流による一義的な変化は粘膜表層の傷害であるが、近年、ビラン性刺激に対してtrefoil peptideが重要な働きをしていることが報告されているので、trefoil peptideの生理的な作用について調べるとともに、逆流発癌モデルのおけるtrefoil peptideの関与についても検討した。得られた研究成果は以下の通りである。(1)ヒト大腸癌と胃癌細胞株を用いた実験で、trefoil peptideファミリーのTFF3がAPC gene-catenin complexとE-cadherin-catenin complexの発現をコントロールしていることが分かった。このことは、trefoil peptideが発癌過程において細胞接着や細胞移動に関連する増殖制御に重要な働きをしていることを示唆している。(2)逆流手術を施行したラットで、術後75週で、従来報告されているような率で胃癌と食道癌を発生させることに成功した。75週以前にラットを屠殺し前癌病変を検索してみると、逆流に最も曝される粘膜深部に特徴的に嚢胞状拡張腺管の形成が観察された。これは胃腺や腸腺の深部からbuddingにより形成されるもので、Wrightらの言うUlcer Associated Cell Lineage(UACL)に相当する。この嚢胞状腺管ではtrefoil peptideのpS2とTFF3が発現していることが免疫組織化学とISHで証明することができた。従来、通常型の胃癌は腺頚部の幹細胞から発生することが報告されてきたが、逆流発癌モデルでは、通常型胃癌とは異なるUACLからの発癌経路があることが示唆された。
