気管支喘息における気管支リモデリングのメカニズムについて

研究概要	今年度は、動物実験および臨床研究を平行して施行した。動物実験は、アスカリス自然感作された綿羊を用い、アスカリス抗原を吸入させないコントロール群および複数回アスカリス抗原を吸入させた群とで好酸球浸潤および気道収縮反応を比較をしている。頻回の人工呼吸器使用等により、感染を併発する動物が観察されるため、最終時の肺組織を観察しながら、感染併発例は除外している。現在症例は少ないものの、複数回アスカリス抗原吸入により好酸球は気管支肺胞洗浄液および組織内に増加し、気道炎症の組織学的所見も認められる。平成7-8年度の科学研究費にて、特異的好酸球遊走阻害剤がアスカリス抗原一回吸入による気管収縮反応および好酸球の気道への浸潤を抑制すると報告してきた。今回は、本剤の薬理学的特性をさらに明らかにするために、現在胃内投与後の血中濃度を測定している。効果作用時間の確認と投与間隔等を検討し、アスカリス抗原複数回吸入に対する効果を検討する。一方、臨床研究では、気管支喘息患者を中心として他の呼吸器疾患患者も含め現在症例を蓄積中である。その中で慢性肺気腫患者の誘発喀痰中に好酸球が増加している患者では、ステロイド短期治療により気道収縮の可逆性も認めらむ、その相関を見いだした。また、咳を主症状とする咳喘息患者でも誘発喀痰中の好酸球数と症状の重症度に関連が認められる傾向がある。このことは、好酸球性気道炎症が、気道病変に広く関与している可能性を示唆している。さらに最近、呼気中Nitric Oxideも測定し誘発喀痰中の好酸球数比率、気道過敏性および各種mediatorとの関連も追求している。最終的にステロイド短期治療による呼吸機能改善度を比較し、気道のリモデリングとの関連を検討する。
