脳磁場計測とDC電位の解析によるヒトのてんかん焦点での興奮および抑制過程の研究

研究概要	本年度は、本研究計画の初年度としてまず主に脳電位に関連して、ヒトのてんかん原性焦点における興奮抑制機構について以下の新知見を得ることができた。詳細は研究発表の第3項目:Ikeda et al:Focalictal DC shifs in human epilepsy as studied by subdural and scalp recording,Brain(in press)に報告した。 1. 従来ヒトの発作時DC電位についてその臨床的意義は明らかにされていなかった。そこで難治部分てんかん患者の焦点切除のために慢性に留置された硬膜下電極から増幅器の時定数を10秒に設定して多数の患者からそれぞれ多数回の発作で検討した。新皮質てんかんおよび内側側頭葉てんかん患者において同様に、発作時DC電位は記録発作中の約80%で明瞭に記録され、その最大部位は従来の脳波記録による発作時焦点と一致あるいはより狭い範囲に認められた。しかし発作開始に必ずしも先行せず、むしろ脳波が低振幅速波化した時点に一致した。これは動物実験での発作時の突発性脱分極変位(PDS)の所見によく一致した。一方抑制系の反映としては発作後半から終了にかけて出現する陽性DC電位がこれを反映すると過去の実験結果からは解釈されており、現在解析中である。 2. 一方、頭皮上電極からの発作時DC電位は出現率が約20%と低かったが、硬膜下電極からの記録と同様に記録所見の特異度は高かった。しかし特に今後の臨床応用には発作時の体動等による人工雑音の解消が課題となる。今後は上記所見に関して、さらに脳磁場変動との比較検討を中心に行う予定である。
