心筋保護におけるアデノシン産生酵素リン酸化・活性化の分子メカニズムとその臨床的意義

研究概要

平成10年度は、培養心筋細胞、ビ-グルイヌを用い、低酸素・再酸素化・虚血・再灌流により生じる心筋不可逆性障害におけるアデノシンの意義を検討した。
ラット培養心筋細胞においてリポフェクチン法を用いccto-5'-rtuclcotidascのcDNAを遺伝子導入する。24-48時間後、ccto-5'-nuclcotidascの蛋白量・活性は増大した。かかる細胞において3時間低酸素3時間再酸素化したところ非導入細胞に比べLDH放出量・トリパンブル-染色法により評価した不可逆性心筋細胞障害は著明に軽減した。このことにより、ccto-5'-nuclcotidaseは心筋低酸素障害に対し保護的に作用することが示された。
一方、ラット、ウサギ、イヌなどのin vivoモデルにおいて、短時間心筋虚血が先行すると、引き続き生じる長時間心筋虚血による不可逆性細胞障害を軽減させる(プレコンディショニング現象、IP)。我々は、麻酔開胸犬において、1)IPによりccto-5'-nuclcotidaseのセリン・スレオニン残基がリン酸化され、その活性が増加すること、2)ccto-5'-nuclcotidase阻害剤にてIPの心筋梗塞縮小効果が消失すること、3)ccto-5'-nuclctoidase活性化には、α_1-アドレナリン受容体刺激-protcin kinase C活性化学が関与すること、4)α_1アドレナリン受容体遮断剤・protcin kinaseが消失することを見出した。
さらに現在、急性心筋梗塞症例においてアデノシンの前駆体であるATPを冠注・静注して、梗塞サイズが縮小するか否かを大規模試験にて検討し、現時点で有意差はないもののATP冠注にて良好な改善を得つつある。
また分子発現ラットを作製注であり、現在ES細胞が完成し、あと2カ月にて過剰発現マウスが完成予定である。かかるラットを用い、虚血・再灌流にて心保護作用が生じるか否かを検討すればアデノシン虚血耐性の関係が明らかになるものと期待される。
