重症慢性活動性EBウイルス感染症症候群の発症機序に関する研究

研究概要

SCAEBV患者10例において、インフォームドコンセントを得た後、各末梢血単核球を得、各種検索を行った。その結果、末梢血単核球表面マーカーの検索では、SCAEBV患者の半数において、CD4陽性細胞の増加を認めた。また、SCAEBV患者は、全例でEBVゲノムを認める一方、対照とした健康人では、EBV抗体陰性例はEBVゲノムを認めず、抗体陽性例のうち1例(10%)でEBVゲノムを認めた。NK細胞活性は、SCAEBV患者10例中6例で、低下を認めた。SCAEBV患者の細胞培養系では全例に細胞変性効果を認めたが、対照では認めなかった。SCAEBV患者2例にて、培養上清中にアデノウイルスを認めた。EBV-CTL機能は、患者由来の細胞株が樹立されず、測定不能であった。EBV抗体陽性健康人は十分なEBV-CTL活性を示し、培養上清中にインターフェロン-ガンマの特異的産生を認めた。
SCAEBVは、T細胞・B細胞増殖性疾患・悪性リンパ腫などを経過中に認める例もあり、非常に予後不良の疾患である。今回の研究で、以下の点が明らかにされた。1)従来の報告にみられるように、SCAEBV患者末梢血単核球においては全例でEBVゲノムの存在を認めた。2)しかし、EBV陽性リンパ芽球株の樹立を認めなかった。3)その主な原因として、培養中の細胞変性効果があげられた。4)一部の例で、アデノウイルスの存在を認めたが、他の代表的なリンパ向性ウイルスの同定は出来なかった。5)免疫学的には、SCAEBV患者において一部に、T細胞数の相対的増加、NK細胞活性の低下を認めた。
以上より、SCAEBV患者では、ゲノムの存在などより、EBVの活性化が病態に反映している事は明らかではあるが、免疫学的には異質性を認めた。今後、疾患診断の同一性および発症からの経過を加味した検討が必要であると思われた。
