急性前骨髄性白血病におけるレチノール酸耐性機序と病態の解明

研究概要

急性前骨髄性白血病(APL)の治療において、all-trans retinoic acid(ATRA)療法後に再発した患者は高頻度にATRA耐性を獲得し難治性となるため、APL治療における大きな問題となっている。
我々は、「ATRA耐性機序がAPL特異的PML/Retinoic acid receptor-α(RARα)キメラ蛋白のATRA結合部位であるE-ドメインの獲得性変異に起因する」との仮説に基づき、APL患者23例のキメラ遺伝子E-ドメイン変異の有無をSSCP/RT-PCR法で解析した。その結果、ATRA感受性患者では変異を認めなかったが、再発時にATRA耐性を獲得した2症例においてのみPML/RARα・Eドメインに点変異(Arg272Gln、Met297Leu)を見い出した。この変異は同一患者の初発時には検出されず、獲得性変異と考えられた。さらに、ATRA耐性患者で見い出された変異を導入したPML/RARα発現ベクターを用いたリガンド依存性in vitro転写活性の解析により、変異PML/RARαのATRA依存性転写活性が著しく低下していることを明らかにした。
本研究のATRA耐性患者2例で見い出されたRARα/E-ドメインの変異部位は、ATRAとRARαの結合に不可欠な役割を果たす部位であること、ATRA耐性を誘導したAPL細胞株においても同一領域に獲得性変異が起こっていることが報告されている。従って、本研究は、APLの病因であるPML/RARαキメラ遺伝子E-ドメインの獲得性変異が臨床上経験するAPL患者のATRA耐性においても耐性獲得機序の一つであることを明らかにした。
