脂肪細胞由来因子(アディポサイトカイン)による血管病発症機構の解明

研究概要

心筋梗塞を初めとする血管病の背景として、過栄養による体脂肪蓄積、特に腹腔内内臓脂肪の蓄積が大きな要因となっている。私達は脂肪組織発現遺伝子の解析から、脂肪組織の中でも特に内臓脂肪がアディポサイトカインと呼ぶべき様々な生理活性分泌因子の遺伝子を多数発現しており、内臓脂肪から分泌されるこれらの因子が直接血管壁に働き血管病発症の一因となっていることを示してきた。本年度はその中で強い血管平滑筋増殖作用を持つHB-EGFと私達が新たに見いだした脂肪組織特異的コラーゲン様分子、アディポネクチンについて検討した。免疫細胞化学的検討により、脂肪細胞におけるHB-EGFの存在を確認した。当教室で開発された血中HB一EGF濃度測定系を用い、体脂肪分布と血中HB-EGF濃度の関係、虚血性疾患患者における血中HB-EGF濃度を測定した。血中HB-EGF濃度は肥満者で高値を示し、特に腹部脂肪面積と正の相関を示した。また虚血性疾患患者では著しい血中HB-EGF濃度の増加が見られ、血管病発症との関連が示唆された。一方血中アディポネクチン濃度は逆に肥満者では低値を示し、虚血性疾患患者では著しく低下していた。in vitroにおいてアディポネクチンは傷害内皮細胞への単球接着や、増殖因子による血管平滑筋細胞増殖を抑制した。それぞれのシグナル伝達経路の検討により、前者はNFκBが、後者はMAPkinaseが関与していることが示唆された。次年度ではアディポネクチンの細胞内シグナルの伝達についてさらに検討を進めていく予定である。
