HB-EGF遺伝子ノックアウトマウスを用いた動脈硬化発症機序の検討

研究概要

動脈硬化症や血管形成術後再狭窄病変の形成には血管平滑筋細胞の形質変換、分化制御の異常が重要な役割を果たしており、平滑筋細胞の形質変換を阻止し増殖を制御することはそれらの治療においても重要な課題である。本研究において私どもは血管壁再構築3次元モデルを開発し、EGF familyのひとつであるheparin-binding EGF like growth factor(HB-EGF)が血管平滑筋細胞に対してEGFよりも強大でPDGFとほぼ同程度の増殖促進作用を有することを確認した。またヒト動脈硬化巣の内膜平滑筋細胞および硬化巣直下の中膜平滑筋細胞で発現していること、さらに血管内皮損傷後再狭窄病変においてHB-EGFの遺伝子発現がPDGFやTGF-αなどの他の増殖因子に先駆け損傷2時間でおこることを示した。さらにHB-EGFは他の増殖因子の遺伝子発現を誘導することも明らかにした。これらの事実は動脈硬化症の発症において極めて早期の段階でHB-EGFが関与していることを示唆するものであり、サイトカインネットワークの発現誘導のtriggerである可能性を強く考えさせた。一方ヒトジェノム計画の一端として脂肪細胞に発現する遺伝子を検討したところ脂肪細胞はHB-EGFを合成しており流血中へ分泌していることが明らかとなった。特に皮下脂肪組織よりも内臓脂肪組織ではHB-EGF遺伝子の発現が強くまた内臓脂肪型肥満患者の血中HB-EGF濃度は高値を示した。血中HB-EGF濃度は血管病変の進行程度と相関関係があることを明らかにした。
