腰椎椎間板ヘルニアの根性疼痛発現におけるアラキドン酸カスケードの役割

研究概要	ラットを用いて腰部脊髄神経根上に尾椎より摘出した髄核を留置することにより発現する神経因性疼痛の指標である痛覚過敏にトロンボキサン(TX)、ロイコトリエン(LT)、シクロオキシゲナーゼ(COX)が関与するかどうかを検討した。TXA_2、LTB_4、COX-2の拮抗薬の硬膜外投与による痛覚過敏の変化を行動薬理学的に観察した。ラットを用いて第4-5腰椎片側椎弓切除術によりL4、5神経根を露出し、尾部の椎間板より摘出した髄核(4mg)をそれぞれの神経根上に移植した。髄核移植により処置後5日目から足部に機械的圧刺激に対する痛覚過敏が出現した。L4-5レベルに硬膜外カテーテルを留置し、溶解剤、TXA_2合成酵素阻害剤、LTB_4受容体拮抗剤、COX-2特異的阻害剤をそれぞれ髄核移植後7日目に投与した。TXA_2合成酵素阻害剤、LTB_4受容体拮抗剤、COX-2特異的阻害剤は投与後早期にには有意な変化はみられなかったが、硬膜外投与後1週目ではいずれも痛覚過敏の有意な改善が得られた。また、組織学的に観察すると、神経根周辺の瘢痕組織は溶解剤投与と比較して炎症細胞の浸潤が軽度であった。熱刺激に対する感受性変化は認められなかった。硬膜損傷を作成したラットではCOX-2特異的阻害剤の投与により下肢運動麻痺の出現がみられた。アラキドン酸カスケードの代謝産物であるTXA_2、LTB_4ならびにアラキドン酸からプロスタグランジン産生に関与するCOX-2が髄核による疼痛発現に関与する可能性があり、神経根周辺の炎症反応に関連することが判明した。これらの阻害剤や拮抗薬の硬膜外投与は日常臨床で使用可能な薬剤となる可能性がある。
