難治性眼免疫病の分子機構

研究概要

難治性眼免疫病はしばしば再発を繰り返し、視力予後不良の事も多い。本年度は、ベーチェット病や原田病などのぶどう膜炎、アレルギー性結膜炎、そしてアデノウイルス結膜炎を中心に取り上げ、その分子免疫学的、分子遺伝学的、分子疫学的発症機構を検索した。その結果、ベーチェット病では我々が日本人で見い出したHLA-B^*5101,MICA-A6との分子相関が、ギリシャ人でも全く同様に見い出された。原田病ではグルコース不耐性が高頻度に見つかり、その成因として急性炎症による産生サイトカインの自己免疫現象が想定された。また、病勢の判定には可溶性細胞間接着因子の重要性が見い出された。これらの基礎データをもとに、我々が中心となって、国際眼炎症学会のぶどう膜炎国際ガイドラインが作成された。
一方、アレルギー性結膜疾患では血清中可溶性細胞間接着因子のうち、ICAM-1,IL-2,VCAM-1が病態形成に重要な意義を有することが明らかにされた。このうち、特に可溶性VCAM-1は春期カタルに特異的に関与し、全身のアトピー性皮膚炎の合併の有無とは相関しなかった。また、少量の全血を用いたヒスタミン遊離試験は原因抗原の確定に有用であることが示された。
アデノウイルス結膜炎ではアデノウイルス3型の新遺伝子型が見い出された。また、アデノウイルスの血清型同定にPCR-シークエンス法を世界で初めて応用し、従来のPCR-RFLP法よりも短時間で、より正確なタイピング成績が得られた。また、分子疫学成績をもとに、過去10年間のアデノウイルス結膜炎の推移を検索した所、アデノウイルスの各血清型は3年から5年毎に主要血清型が変化していた。また、ヘルペスウイルス結膜炎やクラミジア結膜炎も毎年発症していた。
これらの成績から、難治性眼免疫病の発症機構を分子生物学的に研究することの重要性が明らかにされた。
