DHEAおよびメラトニンの血管内皮機能に及ぼす効果

研究概要	加齢と共に低下するステロイドホルモンであるDehydroepiandrosterone(DHEA)は抗酸化作用を持ち、内皮由来一酸化窒素の産生増加作用を持つインスリン様増殖因子の産生を亢進する。冠動脈造影上、異常を認めない男性6名(50〜70才)にてDHEA(25mg/日)3ヶ月間投与前後で内皮機能を表す上腕動脈虚血後の反応性充血時の拡張反応(FMD)を超音波にて検討したところFMDは2.23±1.16%から6.89±0.60%へと改善した(P<0.01)。DHEAにて内皮機能が改善したことから女性における動脈硬化に対するエストロゲン補充療法と同様に男性においてのホルモン補充療法となる可能性が期待される。また、あらゆる心疾患の終末像であり人口の高齢化に伴い増加している心不全の成因として酸化ストレスの増加〜その防御機構の低下が報告されているが、慢性心不全患者ではDHEA血中レベルは心不全の重症度に応じて低下し、酸化ストレスの指標であるThiobarbituric acidrelated substancesと逆相関を有することが明らかになった。DHEA補充療法は慢性心不全患者においては心不全治療薬となる可能性も期待できる。一方、夜間睡眠中に松果体から分泌され日中はほとんど分泌されない日内変動を有するメラトニンは加齢と共に夜間の分泌も低下する。相対的交感神経緊張の指標である心拍変動の低下は心事故の発生と密接な相関があるが、労作性狭心症患者においてはメラトニンの夜間血中レベルは低下しており、その血中レベルに応じて心拍変動が低下することや就眠前のメラトニン内服がそれを回復することを明らかにした。メラトニンの内服が相対的交感神経緊張を緩和し冠動脈疾患を有する患者の心事故発生を低下させる可能性が示唆された。
