悪性リンパ腫にみられる転座プロミスキュイティーの発生機序と臨床病理学的意義

研究概要

AMLやALL、二次性白血病にみられる11q23転座の切断点に存在するMLL遺伝子は、様々な遺伝子と相互転座するpromiscuousな特性をもつが、悪性リンパ腫の特定の亜型発生にかかわるALKとBCL6も転座プロミスキュイティーを示す。転座プロミスキュイティーを示すALKとBCL6遺伝子の関わった相互転座を有する悪性リンパ腫症例を多数例解析し、臨床病理学的に共通の遺伝子異常と病態を有する症例群を亜型化して分類することを目的として、リンパ節生検によりALCLおよびDLBLと診断されたNHL98症例について、本研究ではBCL6転座パートナー遺伝子を解析した。その結果、18/98例でBCL6遺伝子再構成を認め、うち14/18例で5'-partner gene/BCL6-3'融合転写産物の発現が確認できた。IgH以外のパートナー遺伝子としてはMHC class II transactivator(CIITA)、eukaryotic initiation factor 4AII(eif4AII)、ikaros、pim-1およびTFRRの新規パートナー遺伝子を同定した。5'RACE法により転座パートナー遺伝子が検出できた14検体について、LA-PCRを行いゲノム切断点の同定を試みたところ、11例においてBCL6ゲノムの切断点を含むDNA断片を増幅できた。切断点近傍の塩基配列を詳細に調べたところ、(1)BCL6の切断点は体細胞突然変異が起こるとされている領域にほぼ一致すること、(2)切断点近傍にVDJ組換えのシグナル配列であるヘプタマー・ノナマー様シグナル配列やカイ様配列がないこと、(3)IgH以外の遺伝子と転座している検体ではパートナー遺伝子のゲノム切断点近傍の塩基配列にスイッチ領域とのホモロジーが見られないこと、(4)相互転座が確認できた例では、生じた融合遺伝子中に体細胞突然変異時にも起こると報告された塩基配列の欠失や重複があること、(5)新たな転座の機構を示唆するような認識配列は確認できなかったことなどが明らかになった。以上の結果より、BCL6の関わる転座がBCL6遺伝子の体細胞突然変異の過程でのエラーによって生じている可能性が示唆された。パートナーとして検出された遺伝子は、細胞の増殖や分化、さらには癌化に関わるとされる遺伝子であったことから、これらの遺伝子発現の脱制御が悪性リンパ腫発症に関与している可能性がある。また、5'-BCL6/partner gene-3'融合転写産物の発現によるパートナー遺伝子の脱制御機構の関与も否定できない。BCL6転座を介したリンパ腫発症機構を解明し、共通遺伝子異常と病態を有する症例群を亜型分類するためには、今後これらの融合転写産物についての生物学的な機能解析が必要であると思われる。
