研究概要	胃癌の治療は早期発見と外科的手術に頼らざるを得ないのが現状であり、進行・再発胃癌では有効な治療法が確立されておらず、免疫療法などの新しい治療方法の開発が望まれいる。本研究の目的は、胃癌の免疫療法のターゲットとなるべき癌抗原を同定し、さらに臨床応用のための基礎的データを得ることである。具体的には、近年開発されたSEREX法(Serological identification of antigen by recombinant cDNA expression cloning)を用いて、胃癌の癌抗原を同定することを試みた。SEREX法では、癌患者から摘出した癌組織由来のcDNA発現ライブラリーを患者の自家抗体を用いて検索し、反応する癌抗原を同定する。SEREX法の利点は、癌細胞の培養株の樹立を必要としないことであり、あらゆる癌で癌抗原を検索することを可能にした。これまでに、5症例の胃癌でSEREX法による癌抗原の検索を行い、癌患者の免疫系が認識する136種の抗原を同定した。これらの中から、胃癌の免疫治療・診断に有用な抗原を同定するために、抗原に対する免疫反応が癌患者で特異的に誘導されていることを基準として選択することにした。各々の抗原に対する抗体の存在を、44名の胃癌患者群と100名の非癌患者コントロール群で検索した。136種の抗原は、(1)複数の癌患者で抗体が検出されるがコントロール群では全くされない抗原:11種、(2)抗原を同定した患者のみで抗体が検出される抗原:40種、(3)癌患者群と非癌コントロール群の両群で抗体が検出される抗原:85種、以上の3群に分けられた。(1)のグループの抗原が最も期待される抗原である。加えて、(3)のグループの抗原にも癌患者での抗体産生の頻度がコントロール群に比べ有意に高い抗原が約10種あり、これらも有望と考えられる。約20種類の候補の抗原の選択を終え、各々の抗原の発現様式と遺伝子異常の解析を開始した。これらの抗原が患者の免疫反応を惹起した原因を解明することにより、診断・治療における有用性を検討して行く予定である。
