機械刺激・冷刺激に対する痛覚過敏の末梢機構の研究

研究概要

機械刺激・冷刺激に対する痛覚過敏の機構を痛み受容器レベルで解析することを目的として実験を実施し、次のような結果を得た。1)全身性アジュバント関節炎動物モデルにおいて、皮膚-神経標本からC線維侵害受容器活動を記録してBK反応を調べたところ、B2受容体を介するBKの感受性が1000倍近く増大していたので、反応増大の機構の1つとしてB2受容体の発現増大の可能性を考え、後根神経節のB2受容体mRNAの量をRT-PCRにより調べた。炎症症状出現前の接種後1週間ですでにmRNAは増大し、3週目まで増大していた。2)Freundの完全アジュバントの足根関節内投与により単関節炎を誘起したラットでは、接種後10日から2週間してvon Frey毛による機械刺激や、アセトンを用いた軽度冷刺激に対する痛覚過敏が発生した。この痛覚過敏は5週間目まで持続した。3)このような動物における皮膚感覚受容器の冷感受性の変化をしるため、麻酔下の動物でC線維からの単一神経記録をin vivoで行なった。2℃までの冷刺激に対して反応する侵害受容器(C機械冷侵害受容器、C機械熱冷侵害受容器)の割合が対象動物(2/14受容器、14%)に比べてアジュバント関節炎動物では高く(9/18受容器、50%)、またC線維低閾値機械受容器の軽度冷刺激に対する反応が有意に増大していた。C線維低閾値機械受容器の反応温度域はアセトン接触による冷刺激の温度域と近く、従来痛覚には関与していないとされているこの受容器が、アセトン法で検出した冷痛覚過敏に一役を担っている可能性を示唆する。2℃までの冷刺激に感受性の侵害受容器の割合の増加は、より低温の冷刺激に対する痛覚過敏の存在を示唆する。この温度域の冷刺激に対する疼痛反応は目下検討中である。
