小児期発症インスリン依存性糖尿病の発症病因、病態に関する研究

研究概要	平成13年度の研究は以下のテーマで行い、その主な実績を報告する。 1.小児インスリン依存型糖尿病(1型糖尿病)と発症感受性遺伝子小児期発症1型糖尿病患者を次の3群に分けて検討した。(1)乳幼児発症1型糖尿病(5歳未満急性発症)(E群)、(2)5歳以上の急性発症1型糖尿病(A群)、(3)ゆっくり進行する1型糖尿病(S群)。各3群におけるHLA抗原遺伝子、CTLA-4遺伝子多型、NeuroD/BETA2遺伝子多型の検討に加え、内因性レトロウイルスIDDMK1,2-22遺伝子多型について検討した。FNV領域、5'末端(最初のメチオニンcodon)から339(290)および510(461)番目の塩基(nt)に各々(A/G)(A/G)あアミノ醒置換を伴った塩基置換を見いだした。平成13年度はこの2つの多型の意義について検討した。E群において510番目のG/Gホモの頻度が対照に比し有意に高いことを明らかにした(J Hum Genet 46:712,2001)。S群においでは339番目のGの遺伝子頻度が対照に比し有意に増加していた。この意義についても検討を進めている。 2.小児1型糖尿病治療法、コントロールの現状と生活の質(QOL) 小児1型糖尿病の治療の基本は強化インスリン注射療法である。厳格なコントロールのためには、小学生高学年から強化インスリン療法が導入されてきている。わが國44施設の共同研究を行い、登録した約530例を対象にして血糖改善のための介入試験を行った。HbA1c平均値は登録時8.42±1.86、介入後は8.16±1.71に改善した。4回注射法の割合は15歳以上の症例で35%から66%に上昇した(Pediatr Diabetes 2:160,2001)。一方、頻回注射法を含めた治療内容とQOLについての国際共同研究を行った。治療方法に関係なく良好な血糖コントロールがQOLを高めることが明らかになった。思春期女子のHbA1c値は男子より高く、Q0Lも低い事が明らかにされた(Diabetes care 24:1923,2001)。
