研究概要	言語ならびに認知機能の発達障害を主要症状とする自閉症は、ヒト相互のコミュニケーションを成立させる上で欠かせない、相手の感情や意図を察知する能力の障害に病態の本質があると考えられている。我々は研究費補助を受けた期間に、脳活動をリアルタイムで、しかも非侵襲的に測定可能な核磁気共鳴画像法(f-MRI)を駆使して、自閉症者に認められるヒトのコミュニケーションに関わる脳機能レベルでの障害部位の特定を試みた。スキャン中のタスクは、Ekmanらによる表情作成法に準じて作成した、3種の主だった表情('happy'、'disgust'、'fear')の認知タスクとし、30秒毎の"on/off box car"デザインを用い、5分間を一つの表情認知タスクとした。高機能自閉症者5例、健常者9例(両群とも右利き)を研究対象に、各々の表情をon conditionとし、neutral faceをoff conditionとし、タスク下における両群間の脳活動上の相違を、non-parametric解析法(XBAM)を用いて検証した。'disgust'認知に関して、自閉症者は正常比較群に比較して、左側insula、左側inferior frontal gyrus、及び左側putamenにおいて有意に低下した脳活動が認められ、また一方、'fear'タスクにおいても、自閉症例では左側middle frontal gyrusに活動の低下が確認された。しかし、'happy'タスクにおいては、2群間に相違は認められなかった。本研究から、2群間で差異が認められたlimbic systemの一角を成す脳部位が、ヒトの表情認知に関与し、また同部位の異常が自閉症の病態形成に関与していることを示唆された。さらに、自閉症者はすべての表情認知において障害を受けているわけではないことが伺われた。
