顎関節の軟骨破壊とプロテアーゼ(MMP)活性に関する研究

研究概要	顎関節の滑液中に出現する軟骨破壊成分、軟骨基質分解酵素、炎症関連物質を分析し、変形性顎関節症(osteoarthritis;OA)の病態診断基準を確立するために本年度の研究を行った。パンピングマニピュレーションにより上関節腔から滑液を採取し、蛋白濃度を一定に調整後、ELISA法やSDS電気泳動を応用したエンザイモグラム、ウエスタンブロット法を用い、軟骨基質を分解するmatrix metalloproteinase(MMP)の活性を測定した。MMPは活性型、非活性型をエンザイモグラムで測定し、活性型MMP/総MMPの量比をデンシトメーターで測定し、activation ratioとして表わすことで軟骨基質分解の指標とした。その結果、MMP9やMMP3の活性型の出現がOAと関連していること、OA症例ではMMP9のactivation ratioが正常者群より高い値を示すことを見い出した。また、われわれの研究グループではすでに、滑液中のIL-1、TNFαの存在がOAと関連することを見い出しているので、ラット顎関節細胞を使ったin vitroの実験から、これら炎症性サイトカインの一つであるIL-1が滑膜細胞や軟骨細胞及ぼす影響について究明した。その結果IL-1刺激により主に滑膜細胞から活性型MMP9やMMP3が産生されることがわかった。以上の結果から、変形性顎関節症患者の軟骨基質分解にMMP9やMMP3が関与している可能性、その活性型酵素の出現にIL-1が関与している可能性が示唆された。
