口腔内嫌気性菌の誤嚥性肺炎起因性に関する解析

研究概要	口腔細菌が誤嚥性肺炎の原因ではあるものの、その病巣からの細菌の分離は難しい。そのような状況から、嫌気性の歯周病原菌群は、実験的肺炎を成立させるか否か検討してきた。2回から3回の妊娠を終了したBALB/cマウスを実験動物として、口腔内に細菌による肺炎起因性を調べた。Streptococcus pneumoniaeなどは、肺炎を起こし動物を致死させる。一方、歯周病原菌は、10^8オーダーの生菌を直接気管内に接種しても96時間後には、生菌が検出されなくなるものの、肺気管支洗浄液(Bronchoalvelar lavage fluid,BALF)には多くの炎症性サイトカインを放出させることがわかった。次いで歯周病局所から分離した細菌のどのような組み合わせが肺炎を起こすかを明らかにするべき検討を加えた。歯周病原菌としてPorphyromonas gingivalisおよびActinobacillus actinomycetemcomitansを中心としてPropionibacterium acnes,Treponema denticola菌種などと組み合わせ、すなわち混合感染による肺炎起因性を調べた。実験は、単独および組み合わせの生菌を接種後、1)生菌数の消長、2)BALF中のマクロファージの動態、3)BALF中のIL-6、IL-8およびTNFα量を測定した。グラム陽性菌の消長は比較的長かった。歯周病原性グラム陰性菌の単独および組み合わせは、強い炎症性サイトカイン誘導能を認めたが1週間後には生菌が検出されなくなった。結論として、歯周病原性細菌は、本実験マウスを肺炎で死に至らせることができないものの、炎症性サイトカインの産生を誘導することがわかった。
