心不全モデルを用いた治療の開発

研究概要	実験動物モデルを用いて、従来の細胞内カルシウム(Ca)上昇により強心作用を発揮するCaモビライザーおよび収縮タンパク感受性増強により強心作用を惹起するCaセンシタイザーの作用がウサギ容量負荷心不全モデルおよびアシドーシスでどのように修飾されるかを明らかにし、心不全治療薬の開発に必要な基礎的データを得ることを目的に実施された。実験としては、新しいCaセンシタイザー(レボシメンダン、OR1896、UD-CG212Cl、ピモベンダン)をもちいて生理的条件下におけるCaセンシタイジング作用の特徴づけを行った。さらにアシドーシスにおけるCaセンシタイジング作用の修飾、およびウサギ容量負荷それらの薬物の強心作用の機序の解明を行った。多くのカルシウム・センシタイザーは、収縮タンパクCa感受性増強作用とcAMP PDE III阻害作用をもつ。本研究では、ヨーロッパですでに臨床的にうっ血性心不全の注射による治療薬として使用されているレボシメンダンの強心作用機序についてエクオリン負荷ウサギ乳頭筋標本およびindo-1負荷ウサギ単一心室筋細胞を用いて実験を行った。レボシメンダンは、両標本において収縮タンパクCa感受性増強作用を惹起したが、レボシメンダンによるカルシウム感受性増強作用はムスカリン受容体アゴニストであるカルバコールにより遮断された。この結果は従来の考え方、すなわち、cAMPはトロポニンIリン酸化と関連した収縮タンパクCa感受性減少とホスホランバンリン酸化によるSRカルシウム・ポンプであるSERCA2の活性化によりpositive lusitropic effect(弛緩促進効果)を惹起するという見解とは相容れない。本研究結果は、cAMPがレボシメンダンによるCa感受性増強作用に関与していることを示唆する。この細胞レベルにおける機構は現在のところ不明だが、従来観察されていなかった新知見である。これと同様の作用がOR1896、UD-CG212Clでも観察され、新しいCaセンシタイジングの機序として注目される。
