IL-12とIL-18産生によって引き起こされる移植拒絶反応及びGVHDについて

研究概要

白血病をはじめとする血液細胞・免疫担当細胞の悪性腫瘍に対しては徹底した化学療法と骨髄移植が、また、一部の免疫不全に対しても骨髄移植は有効な治療方法である。しかし、通常の骨髄移植にはドナー・レシピエント間の多少の組織適合抗原の不適合が黙認されている。その結果、基礎疾患は完治しても多様な臓器病変を伴う移植片対宿主病(GVHD)をおうおうにして随伴する。当該研究において、急性GVHDの重症度と患者血中IL-18値が正に相関することが明らかになった。また、マウスモデルにおいても同様結果が得られた。ところで、IL-18は生理活性を欠く前駆体として貯蔵され、適正な刺激により活性化された切断酵素が前駆体を切断して活性型IL-18が分泌される。急性GVHDにおけるIL-18分泌機構を検討したところ、Fas/Fasリガンドにより活性化されるcaspase-1が必須であることが判明した。さらに、caspase-1欠損マウスでは、急性GVHDを誘導しても血清IL-18上昇が認められないばかりでなく、種々の臓器病変も著しく軽減することから、caspase-1阻害を標的にした新たな臨床治療応用の可能性が示唆された。
