珪酸化合物曝露による作業関連疾患予防のためのチェックリスト作製の試み-とくにHLA型との関連について

研究概要

ギリシャ・ローマ時代には既に、珪酸silicaの粉じんを吸収すると人に健康障害が起こることが知られていたという。1914年にBramwellらが、珪肺症とSystemic sclerosis(SSc)の合併を報告して以来、珪酸や珪酸塩化合物曝露と自己免疫疾患の合併が知られる様になった。
私どもが、珪肺症患者の血清を調べたところ、その70%以上に自己抗体が検出された。新しく確認された自己抗体には、topoisomerase I,β-tubulin,Fas,Fas ligand等に対する抗体や、ANCA,pemphigus抗体等も含まれる。これらの自己抗体を持つ個人の遺伝的素因について調べるため(本人のプライバシーの侵害や社会的不利益等を伴う可能性が今のところ考えられない)HLA class IIについてのタイピングを行ってみると、特定のalleleやHaplotypeと関連の強い自己抗体が多数あることが判明した。抗-topoisomerase I自己抗体陽性者を例に挙げると、日本人ではHLA DQB1に^※0402など^※04をもつ人が多く、白人では異なったalleleと相関すること、これらの共通点を調べると、DQB1ドメインの14番目のアミノ酸がメチオニン、30番目がタイロシン、57番目がアスパラギン酸、77番目がスレオニンである場合であることが判明し、報告した。
以上の如く、新しく検出された自己抗体各々についても詳細な解析を行っており、これらをリスト化することにより、就業時の適性判断と作業関連疾患予防に役立ちうるものと考える。
