伊東(星)細胞の肝発癌過程における前癌性病変及び癌細胞とのクロストーク

研究概要	本年度の研究成果は以下のごとくである。 (1)コリン欠乏アミノ酸置換食をラットに投与すると、肝細胞壊死-再生が発生し、炎症が持続する。この持続する炎症の結果数々のサイトカインが産生され、これによって伊東(星)細胞の活性化が起こり、線維化が生じる。4ヶ月程度経過すると線維隔壁で囲まれた前癌性病変の出現を認める。肝細胞壊死-再生が持続している状態では、Kupffer細胞や活性化した伊東細胞がTGF-beta1を活発に産生し、ますます線維化が進行する。正常肝細胞は、TGF-beta1により再生が抑制され再生が起こりにくい状態となるが、これら前癌性病変の細胞集団は、すでにTGF-beta1の増殖抑制機構から逸脱していることが判明した。 (2)ラットにブタ血清を週2回8週間投与すると、肝細胞死を発生することなく、伊東細胞の活性化がおこり、肝線維化が発生する。このように、肝細胞壊死-再生といった細胞周期を動かさず、肝線維化を発生させた線維肝にコリン欠乏アミノ酸置換食を6週間投与する群と、コリン欠乏アミノ酸置換食を正常肝を持ったラットに投与する2群間で、肝線維化と前癌性病変の関連性を評価すると、既存の線維肝をもったラットでは、同じ期間コリン欠乏アミノ酸置換食を投与して細胞死-再生を発生させても、数も大きさも有意に増加して前癌性病変が認められた。すなわち、肝線維化は発癌に促進的に働くことが証明された。 (3)肝癌細胞と伊東細胞を共培養すると、伊東細胞が癌細胞に向かって遊走することが判明した。これは、癌細胞周囲に線維化を作りだし、癌細胞の更なる増殖や浸潤を食い止めるための生理学的な反応かは、今後の検討が必要である。
