C型肝炎ウイルスと肝外病変(特に口腔前癌病変と口腔癌の発症について)

研究概要	【目的】C型慢性肝疾患患者からの肝細胞癌の年間発生率は報告されているが,一般住民におけるHCV感染者での肝細胞癌あるいは肝硬変の年間死亡率を明らかにした報告はない。我々は,1990年から1999年にかけてC型肝炎ウイルス(HCV)高感染地区におけるHCV感染者の予後について,cohort studyを行ない,肝硬変や肝癌で死亡した症例の関連因子について検討した。【対象・方法】対象は,1990年〜1994年までに行なわれたH町での検診参加者3708名中(H町住民全員を対象に実施),1999年8月までに死亡が確認された182名(M/F:108/74名,平均死亡年齢:75.6±11.7歳)である。182人の死亡者は,過去に行われた検診によって全員HCVならびにHBV感染の有無が判明していた。182人全員の死因調査を行った。【結果】HCV高感染地区の死因調査によって,HCV抗体陽性例あるいはHCV RNA陽性例における肝細胞癌あるいは肝硬変死亡例は,43.7%もしくは54.9%と高率であった。また同町の肝細胞癌死あるいは肝硬変死に関与する因子は,多変量解析の結果,HCV持続感染とALT値であることが明らかとなった。【結論】今後,同町の住民の高齢化に伴い,高齢者のHCV感染者における肝細胞癌発症の厳重な経過観察を行ないたい。
