各種神経変性疾患における髄液中3-nitrotyrosineの検討

研究概要	対象・方法未治療のパーキンソン病(PD)患者15例(66.1±9.2歳)、アルツハイマー型痴呆(ATD)患者14例(68.9±4.2歳)、筋萎縮性側索硬化症(ALS)患者14例(63.2±7.6歳)および正常対照群15例(65.1±7.8歳)を対象とし,腰椎穿刺で得られた脳脊髄液(髄液)中の内因性一酸化窒素合成阻害物質であるasymmetrical dimethylarginine (ADMA)およびL-arginine (Arg)濃度を高速液体クロマトグラフィー・クーロケム電気化学検出器を用いて測定した. 結果 1.髄液中ADMA濃度(nM)は、正常対照群26.8±7.7、PD群24.9±8.8、ATD群13.7±6.2、ALS群14.9±7.2で、ATD群およびALS群で正常対照群に比し有意に減少していた(各々、p<0.001). 2.髄液中Arg濃度(μM)は、正常対照群39.3±5.2、PD群41.6±9.7、ATD群44.0±8.0、ALS群42.3±8.8で、4群間に有意差を認めなかった. 3.ATD群において、痴呆スケール(Mini-Mental State Examination score)と髄液中ADMA濃度との間に有意な正の相関を認めた(r_s=0.58、P<0.05). 結論 ATDおよびALSの病態に内因性一酸化窒素合成阻害物質が関与することが示唆された.特に、ATDでは発症機序のみでなく病期進行にも深く関与することが示された.
