冠攣縮性狭心症の発症機構に関する分子生物学的研究

研究概要	我々は冠攣縮性狭心症において、血管内皮型一酸化窒素合成酵素(eNOS)遺伝子に2つの重要な遺伝子変異(Glu298Asp,T-786→C)を見出し、それが冠攣縮と有意に連関していることを報告している。これらの遺伝子変異が単なるマーカーであるのか、それともその変異自体が機能異常を引き起こすのかは極めて重要な検討課題であり、その解明は冠攣縮の病態解明の新たなる展開と、他の疾患の病態解析へと繋がる可能性を秘めている。Glu298Aspに関する機能解析では、変異蛋白の酵素学的な機能低下は認められなかったが、蛋白の安定性が低下していることが見出された。何らかの未知の酵素が変異蛋白の分解を促進している可能性がある。一方、T-786→Cに関しては、我々は既にLuciferase reporter gene assayにて変異遺伝子の転写活性が低下することを報告している。さらに、その変異部位に結合する蛋白が、転写因子replication protein A1(RPA1)であることを見出している。最近、我々は、このRPA1が他の複数の遺伝子においても、eNOSと同様な遺伝子配列に結合することを見出している。RPA1はDNAのreplication、repair、recombinationに作用すると言われていたが、おそらく種々の遺伝子においてrepressorとして多彩な働きを有するもと想像される。我々は冠攣縮と連関する他の遺伝子変異を検索している。特に酸化ストレスに関係すると言われているparaoxonase(PON)を中心に検討を進めている。PONにはGln192Argが存在し、その変異体は抗酸化ストレス作用が弱いと報告されている。我々は冠攣縮との関係を検討したが、その結果、有意の連関を認めた。冠攣縮の発症は、eNOS以外の酸化関連遺伝子にも強く関係していることが示され、冠攣縮の発症機構における遺伝的要因の存在の重要性が今後益々問われるものと思われる。
