アトピー性皮膚炎の紫外線療法に関する臨床的ならびにモデルマウスによる研究

研究概要	アトピー性皮膚炎は、皮膚科領域では最も多い疾患であるが、近年とくに患者数の増加と重症化、遷延化の傾向がうかがわれる。アトピー性皮膚炎の治療はステロイド外用が主体であるが、われわれは過去10年以上にわたって、補助療法として数多くの重症患者に、8-methoxypsoralen内服投与とUVA照射によるPUVA療法ないしはUVB単独照射療法を行い良好な結果を得てきた。今回、アトピー性皮膚炎の光線療法を確立する目的で、自験108例における治療内容と臨床効果、検査結果への影響などを解析した。患者は、20才代が半数以上を占め、男女比は7:2であった。すべて入院治療とし、8-MOP30mg投与後にUVAを2〜6J/cm^2照射した。1週間に3回治療し、80%の患者が計10〜30回治療、UVA総量約100J/cm^2をうけた。治療効果を重症度スコアの変化でみると、PUVA療法開始後4週間(12回治療)まで直線的に症状が改善し、以後はプラトーに達した。症状の改善に伴って血清LDH値が減少したが、IgE値、末梢血好酸球数には変化がみられなかった。副作用で治療を変更した患者はなかった。次に、アトピー性皮膚炎のモデル動物とされているNC/Ngaマウスの皮膚炎に対するPUVA療法の効果を検討した。週に3回、計12回、PUVA単独の治療を行った。3回の治療で効果が発現し、中止4週後も効果が持続していた。さらに、発症前からのPUVA療法で発症予防効果もみられた。結論として、内服PUVA療法は成人の重症アトピー性皮膚炎に対して有用性の高い治療法と考えられる。PUVA療法の効果は、アトピー性皮膚炎のモデルマウスによっても確認された。
