大動脈瘤治療のための新しいニチノール性大動脈ステントグラフトの開発に関する研究-細径化と柔軟性の改善を目指して-

研究概要	平成11年度に作製した一本のnitinol wireを組紐状に編み大動脈の形状にあわせて円筒型とするシステムのin vitroでの強度試験を実施した。径2-3cmの円筒形の場合は0.30mm径のニチノール線でメッシュ間隔4-5mmで編んだ場合にステンレスワイヤー性の同径のZステントとほぼ同様の拡張力が得られることがわかった。またin vitroの伸縮試験で約8年以上で明らかな断裂や破損は認めず、現在さらに長期間の検討が持続している。同時にシームレスの円筒状のポリエステル膜を種々作製し最終的に厚さ0.1mm、porosity500-1000の膜がシステムの細経化には最適であることを確認した。同様のステントグラフトで径1cm,2cm長のものを作製し成犬2頭の大動脈に留置し開存性、生体適合性を検討した。約6カ月の観察で開存性は良好であった。この研究中に同様の材質によるステントの良好な生体適合性の報告があり、またニチノールやポリエステル膜のの生体適合性はこれまでに十分な実績があることから、手術が極めて危険でかつ困難な破裂の危険の大きいあるいは破裂した大動脈瘤および総腸骨動脈瘤例にたいし臨床応用した。いずれも動脈の屈曲や狭小化のために通常のZステントによるステントグラフトでは留置が困難と考えられたものである。従来のシステムに比し18Fのシースカテーテルで留置可能であるという利点が確認された。さらにこれらの物理学的性状や動脈瘤の除去能あるいは減圧能を検討するためのモデル回路系(シリコン性の大動脈瘤モデル、接続ビニール製チューブ、加圧ポンプ、動脈瘤内圧測定器による複合体)を作製し、種々の条件下でのパルス加圧による最適システムの検討を施行中であるがこの実験系の構築に長時間を要し、この点については今後検討を継続する予定である。
